Primary Care

Primary care is the provision of integrated, accessible health care services by clinicians who are accountable for addressing a large majority of personal health care needs, and developing a sustained partnership with patients.

A primary care practice serves as the patient’s first point of entry into the health care system and as the continuing focal point for all needed health care services. The Chesapeake Healthcare Center, LLC provides patients with ready access to their own personal physician, or to an established back-up physician when the primary physician is not available.

Chesapeake Healthcare Center will provide internal medicine to adult patients only.  Your primary care physician serves as the entry point for substantially all of your medical and health care needs. He or she is your advocate in coordinating the use of the entire health care system to benefit you and takes continuing responsibility for providing your care. Primary care includes health promotion, disease prevention, health maintenance, counseling, patient education, diagnosis and treatment of acute and chronic illnesses. By regularly seeing a primary care practitioner, you can identify and control health risk factors before they become problems. Your primary care practitioner encourages you to take charge of your health and is a trusted partner in an environment where you can feel comfortable and known.
